APPLICATION PROCESS
If you are interested in coming on our 2011 Momentum Missions trip, please contact Pastor Lucas Forsthoff, Momentum Missions Director, at 248.887.4200 or at lucas@momentumcornerstone.com.
If you are interested in applying for Momentum Missions, please complete the following steps.
To take advantage of a substantial application discount…
Complete and submit the online application by August 20th, 2011.
In conjunction with the completed application, please submit the application fee of $100, which will be applied toward the cost of the trip. To receive the application discount your application and application fee must be submitted or postmarked no later than August 20th, 2011. Please see the specific details below.
If applying after August 20th, 2011…
Complete and submit the online application.
In conjunction with the completed application, the cost of the trip will increase by $120 as a late fee.
ADDITIONAL INFORMATION FOR ALL APPLICANTS
Please note all application fees are non-refundable.
Passport fees are extra and are the missionary’s responsibility.
All checks must be made payable to Cornerstone Church and should be dropped off to Pastor Lucas Forsthoff at Cornerstone Church or mailed to:

Financial Deadlines:
50% of trip cost is due by July 20th, 2011.
100% of the trip cost is due by August 20th, 2011.
Paperwork Deadlines:
All paperwork is due by August 20th, 2010.
